How Do I Get OPWDD Services?
Your child may be eligible for services through the NY State Office for People with Developmental Disabilities (OPWDD), including respite, community habilitation, after school programs, summer camp, behavior management, family reimbursement, and more. A Care Manager can help you get these services in place for your child.
In order to get most services through OPWDD, you need to contact a CCO, have your child evaluated, go through OPWDD’s Front Door process, and get a Care Manager.

Eligibility Process
Request Evaluations
You or your CCO request current psychological and psychosocial evaluations through an Article 16 clinic or other organization.
Also ask your child’s doctor for a medical evaluation (annual physical examination report, including developmental disability diagnosis). Additional documentation may be required by OPWDD for eligibility.

Your CCO helps submit the evaluations to OPWDD to determine your child’s eligibility.
Front Door Process
Front Door Information Session
To get services you must attend a Front Door information session, an orientation on OPWDD services. Find a Front Door session online:

Front Door Interview
After establishing eligibility and attending a Front Door session, an OPWDD representative will call you to complete an interview about your child’s needs and the services you would like to apply for.
Get a Care Manager
The CCO will assign a Care Manager who will help you manage your child’s OPWDD services.
