Life Adjustment Center, Inc. operates as a nonprofit provider agency under the jurisdiction of the New York State Office for People With Developmental Disabilities.
In order for families or individuals to participate in the services available, they must follow a process that determines eligibility, evaluates needs, and then identifies and gives access to services.
The Front Door
The Front Door involves the initial process for determining eligibility, assessing strengths, identifying support needs, and planning authorization.
Care Coordination
The next steps involve identifying a Care Manager who will work with you to coordinate your supports and services, based on a “Life Plan” that is unique to you.
Care Manager
There are several Care Management options based on your specific needs: Home Health Care Management, Basic Home and Community Based Services, and people living in Downstate New York FIDA-IDD (Fully Integrated Duals Advantage for Individuals with Intellectual and Developmental Disabilities).
